Add to Pipeline: skip contacts already in that pipeline

The "Add to Pipeline" workflow action always creates a new opportunity, even when the contact already has an open opportunity in that pipeline, which creates duplicate cards. Add an option to skip contacts that already have an open opportunity in the pipeline (or make that the default).
